we assist with referrals to numerous agencies whether it be housing, utilities, food pantries etc.

"HIRE" A FAMILY MEMBER OR FRIEND TO CARE FOR YOU AND THEY'LL GET PAID!

STEP 1-GET ASSESSED

A nurse will visit you at home to determine if you’re eligible for the CDS program. You'll find out how many weekly hours of care you can get.

STEP 2-CHOOSE A CAREGIVER

You choose a caregiver: your daughter, son, relative, or friend. We come to your home to enroll you in the program.

STEP 3- YOU GET CARE— THEY GET PAID

You get ongoing care from a person who loves you. They get peace of mind because they’re getting paid for the care they give.

WHO'S ELIGIBLE FOR CONSUMER DIRECTED SERVICES?

• The consumer must be receiving or eligible for Medicaid.

• The consumer must be at least 18 years of age.

• Have a physical disability.

• Be able and willing to recruit, select and manage their attendants

• Be seeking or living in an independent living situation.

• Be able to self-direct their services.

• The attendant must be 18 years of age.

• A friend or family member may be hired as an attendant. Exceptions to that rule include spouses and anyone under the age of 18.

• The attendant must clear a background check through the Missouri State Family Care Safety Registry.
